Authentication bypass via SAML assertion replay

Authentication bypass in Dropbox Samly 0.3.0 and later allows remote attackers to log in as another user via replayed SAML assertions. The decode_idp_auth_resp/3 function in lib/samly/helper.ex passes IdP responses to esaml_sp:validate_assertion/2, which uses a default duplicate detector that always returns ok instead of rejecting reused assertions. Attackers need a valid SAMLResponse from network capture, browser history, or logs; replay succeeds only until NotOnOrAfter expires, but each submission creates a new session as the assertion's subject.
