Boot-time crash via unchecked AMD IOMMU device index

Out-of-bounds read in the AMD IOMMU subsystem in affected Linux Kernel releases allows local users to trigger a boot-time crash. __rlookup_amd_iommu() indexes rlookup_table with an unchecked PCI device identifier, after which check_device() dereferences adjacent slab data as an amd_iommu structure. The crash requires an AMD IOMMU configuration whose IVRS omits a device BDF and a tightly allocated lookup table, producing a general protection fault that prevents the system from booting.
