Local I/O hang via stale cancellation flags

Denial of service in Linux Kernel 6.14.6 through 6.15 allows local users to hang ublk command cancellation during recovery. During recovery, per-command canceled flags are cleared only after every queued operation is fetched, so a partial fetch leaves flags set and makes ublk_cancel_cmd skip io_uring_cmd_done. Triggering requires a ublk server to die after fetching a nonempty proper subset of queued operations; the resulting outstanding commands remain uncanceled.
