Process crash via missing segment-template guards

NULL pointer dereference in GPAC master HEAD allows attackers to crash MP4Box by processing a crafted Smooth Streaming manifest. smooth_parse_stream_index() processes c elements by dereferencing set->segment_template->segment_timeline->entries even when a missing Url attribute leaves segment_template unset. Processing the manifest with MP4Box -mpd is required, and successful exploitation terminates that process with SIGSEGV.
