Cross-tenant PIN lockout via global throttle key

Denial of service in OpenReception before 1.0.4 allows remote attackers to block a patient's PIN login across tenants. PIN challenge endpoints increment and consult a central challenge_throttle row keyed only by emailHash, without a tenant identifier. Exploitation requires the same email hash in two tenants and challenge access in one; repeated failed responses impose escalating lockouts on the other tenant.
