Cross-resource read and write via missing authority checks

Authorization bypass in Pathling Server before 2.0.0 allows remote authenticated users to read or modify unauthorized FHIR resources. Typed search, update, and related providers enforce only coarse operation authorities and omit the matching per-resource read or write check. A token with search or update authority is sufficient to disclose or overwrite resource families outside its assigned scope in an authentication-enabled deployment.
