Password-hash weakening via predictable salt generation

Weak salt generation in Crypt::SaltedHash 0.09 and earlier allows attackers to accelerate password guessing via predictable salts. The __generate_hex_salt function selects each hexadecimal salt character with Perl's noncryptographic rand function instead of a system randomness source. Exposure requires an application to use automatically generated salts; predictability reduces resistance to precomputed guessing rather than directly disclosing plaintext.
