Remote service interruption via unlimited HTTP/2 streams

Resource exhaustion in Netty HTTP/2 before 4.1.135.Final and 4.2.0 through 4.2.14.Final allows remote attackers to interrupt service. DefaultEndpoint initializes its active-stream limit to Integer.MAX_VALUE while Http2Settings omits SETTINGS_MAX_CONCURRENT_STREAMS unless the application supplies one. An unauthenticated client can retain hundreds of thousands of streams on one TCP connection, consuming per-stream objects and flow-control state until service availability degrades.
