Remote crash via null host fallback dereference

Null-pointer dereference in Envoy 1.37.0 through 1.37.4 and 1.38.0 through 1.38.2 allows remote attackers to crash the proxy. The REQUESTED_SERVER_NAME formatter processes a null :authority or x-envoy-original-host value without checking it when a host-fallback option is selected. Exploitation requires a HOST_FIRST or SNI_FIRST-style log format and a request lacking the selected host header; the resulting segmentation fault interrupts the Envoy service.
