Authenticated proxy crash via batched gRPC responses

Use-after-free in Envoy's HTTP ext_proc response handling allows remote authenticated users controlling its gRPC service to crash the proxy. AsyncStreamImpl::onData() continues its response loop after callbacks_.onReceiveMessageRaw() destroys the callback object, then dereferences it for a later batched response. Exploitation requires the HTTP ext_proc filter, control of its configured service, and a request reaching the filter; the bounded impact is an Envoy process crash.
