Unauthenticated UCP login via hard-coded credentials

Hard-coded credentials in FreePBX userman 15.0.42 through 16.0.44 and 17.0.1 through 17.0.6 allow remote attackers to access UCP. The optional generic-template setup creates a UCP template account with a fixed sample password and leaves that password usable until it is changed. Exploitation requires an authenticated administrator to have run the setup and retained the initial password; a successful login grants only the template account's configured UCP permissions.
