Realm administration via hardcoded role mapper injection

Privilege escalation in Red Hat build of Keycloak 26.4 allows remote authenticated users to gain full realm administration. The OIDC hardcoded-role protocol mapper accepts realm-management.realm-admin for a client and inserts that role into generated tokens despite scope restrictions. The attacker needs manage-clients permission; that role is sufficient to create the mapper, authenticate through the client, and call realm administration APIs.
