Kernel crash via NULL uctx error cleanup

NULL pointer dereference in the Linux kernel's ocrdma driver allows local users to crash the kernel through a protection-domain error path. In ocrdma_copy_pd_uresp, two failure branches pass pd->uctx to ocrdma_del_mmap before that field is initialized instead of using the valid local uctx. Exploitation requires low-privilege local access, the ocrdma driver, and a response-copy failure during protection-domain allocation; the documented consequence is a kernel crash.
