Kernel crash via double RBD disk teardown

NULL pointer dereference in the Linux kernel RBD driver allows local users to crash affected releases during RBD image mapping. In do_rbd_add(), failed device_add_disk() handling calls rbd_free_disk() and later repeats disk cleanup through rbd_dev_device_release(), leaving blk-mq state invalid. Triggering the crash requires local access to map an RBD image through add_single_major while device_add_disk() fails after device_add() succeeds.
