Guest-to-host code execution via duplicate descriptor reuse

Use-after-free in Cloud Hypervisor 21.0 through 51.1 allows guest OS users to execute code on the host via duplicate virtio-block chains. The async virtio-block submit path accepts two in-flight chains with the same head_index, so completion matches the wrong request and frees a bounce buffer still used by kernel I/O. Asynchronous virtio-block I/O is enabled by default, and an unaligned read or write provides memory corruption in the VMM process.
