TOTP bypass via existing-secret QR disclosure

Improper authentication in FileRise before 3.12.0 allows remote authenticated users to bypass TOTP and obtain full sessions. The TOTP setup controller accepts password-only pending_login_user sessions, and UserModel::setupTOTP decrypts an existing secret and embeds it in the returned QR PNG. Knowledge of the account password and preconfigured TOTP are required; the exposed long-lived secret permits repeated login without the authenticator device.
