Memory exhaustion via retained connection timers

Memory leaks in connection cleanup in Prosody before 0.12.6 and 1.0.0 through 13.0.4 allow remote attackers to exhaust server memory. The client-to-server module and epoll backend leave timeout timers registered after streams disconnect, while authenticated notification and event-hook paths retain response handlers or service objects. Unauthenticated connections can trigger the first two leaks, and concurrent connections can compound retained resources until the service is interrupted.
