Task hangs via leaked device-mapper requests

Denial of service in the Linux Kernel device-mapper allows local users to hang tasks indefinitely through I/O timeout fault injection. The request-based dm path calls blk_should_fake_timeout even though dm has no timeout handler, so an injected timeout leaves the request uncompleted. Reachability requires permission to configure kernel fault injection, a dm device backed by an iSCSI slave, subsequent I/O, and removal of the iSCSI session.
