Host hard-lock via inaccessible-device ATS invalidation

Denial of service in the Linux kernel's Intel VT-d IOMMU allows local users to hard-lock a host during faulty PCIe device teardown. The dev-IOTLB invalidation path uses pci_dev_is_disconnected() to decide whether to issue an ATS invalidation, but that check misses fault-induced PCIe link-down and submits the request to an inaccessible device. Reachability requires scalable-mode Intel VT-d with ATS/PASID and a faulted or link-down PCIe device during a teardown path such as VM resource release.
