LUN removal hang via undrained reset commands

Resource-release error in affected Linux Kernel versions allows local users to hang configfs LUN removal during SCSI error recovery. tcm_loop_target_reset() returns SUCCESS without draining in-flight commands, so queuecommand reuses a scsi_cmnd and zeroes se_lun and lun_ref_active before deferred completion releases the prior LUN reference. The path requires local access and tcm_loop SCSI error recovery; the leaked reference leaves transport_clear_lun_ref() waiting indefinitely and blocks LUN unlink in D-state.
