Tool approval bypass via unauthenticated allowlist modification

Authentication bypass in the PraisonAI gateway before 4.5.128 allows local users to modify the agent tool approval allowlist. The /api/approval/allow-list handler treats an unset auth_token as authorized and inserts any submitted tool_name into ExecApprovalManager's permanent allowlist. With the default loopback binding and no token configured, a local process can make shell, file-write, or code-execution tools run without human review for the gateway process lifetime.
