Authenticated server crash via Office zip bomb

Denial of service in Gophish 0.12.1 and earlier allows remote authenticated users to crash the server via a crafted Office attachment. ApplyTemplate() in models/attachment.go reads every uncompressed Office archive entry with ioutil.ReadAll() without enforcing an expanded-size limit. A User-role account is required; one upload can exhaust memory until the operating system terminates the process, interrupting all service.
