Cross-organization sign-in via missing scope enforcement

Authorization bypass in ZITADEL before 3.4.9 and 4.0.0 through 4.12.2 allows remote attackers to sign in across organizations. Device authorization requests and all Login V2 and OIDC API V2 endpoints omit validation that the session user belongs to the organization enforced by the requested scope. Exploitation requires an application to enforce an organization scope; role- or authorization-based application controls remain unaffected.
