Global profile creation via unchecked user identifier

Authorization bypass in MantisBT 2.28.0 through 2.28.1 allows remote authenticated users to create unauthorized global profiles. account_prof_update.php accepts the request-controlled user_id value ALL_USERS without enforcing manage_global_profile_threshold, while personal-profile creation checks only add_profile_threshold. Possession of add_profile_threshold is sufficient to write a global profile despite the missing global-management privilege.
