Unauthenticated code execution via calculation-field eval injection

PHP code injection in Everest Forms Pro 1.9.12 and earlier allows remote attackers to execute arbitrary PHP code through public form submissions. The Calculation Addon's process_filter() wraps submitted string-field values in single quotes, appends them to generated PHP, and sends the result to eval() without PHP-context escaping. No authentication is required, but the targeted form must use Complex Calculation; successful exploitation provides server-side code execution.
