MFA bypass via inverted bearer-token query

Authentication bypass in ApostropheCMS 3.0.0 through 4.27.1 allows remote attackers with valid passwords to access accounts without MFA. The getBearer function tests requirementsToVerify with MongoDB's not-equal operator, so nonempty arrays of unfinished requirements match. Exposure requires login-totp or a custom afterPasswordVerified requirement; accepted tokens grant the victim's CMS API access.
