Account takeover via failed-check expiration resets

Insufficient expiration in ZITADEL before 3.4.8 and 4.0.0 through 4.12.1 allows remote attackers to take over accounts with expired codes. humanVerifyPasswordlessInitCode checks expiry against the write model's mutable ChangeDate, so each failed check moves the reference time and reopens the validity window. Exploitation requires knowledge or enumeration of an expired code; repeated failures eventually invalidate it, while success registers an attacker-controlled passkey for the victim account.
