Uncontrolled ascent via unchecked armed mode transition

Logic flaw in PX4 Autopilot 1.12.x through 1.15.x allows attackers to cause uncontrolled ascent via a radio mode-change command. Commander::handle_command accepts VEHICLE_CMD_DO_SET_MODE transitions to Manual while armed without calling check_if_throttle_low, so raised throttle is applied immediately. The vehicle must have landed but remain armed during COM_DISARM_LAND with throttle raised; the resulting flyaway can cause a crash and property damage.
