Proxy crash via stale gRPC request state

Use-after-free in affected Envoy releases allows remote authenticated users to crash the proxy through the global rate limit filter. The shared safe gRPC client retains completed request-phase state and accesses that stale state when the response-phase request fails immediately. Exploitation requires both limit phases in one filter, apply_on_stream_done, and a direct rate-limit service failure; the bounded consequence is an Envoy process crash.
