Stored XSS via unescaped questionnaire answers

Stored XSS in OpenEMR before 8.0.0 allows remote authenticated users with Forms administration access to execute JavaScript in viewers' browsers. QuestionnaireResponseService.php inserts extracted questionnaire answer values directly into an HTML span without text encoding. A victim with permission to view the patient encounter or visit history triggers the stored script, exposing that browser session and accessible medical data.
