Pgvector password disclosure via raw configuration logging

Information disclosure in Llama Stack before 0.4.0rc3 allows local users to read a pgvector password from initialization logs. The PGVectorAdapter.initialize method passes the full self.config object to log.info, whose representation includes the cleartext password rather than a redacted copy. Exposure requires the PGVector adapter to be configured and the attacker to obtain its initialization output; the resulting access is limited to the disclosed database credential.
