Cross-patient data access via omitted ACL checks

Missing authorization in OpenEMR before 8.0.0 allows remote authenticated users to read and modify other patients' records via REST. The document and insurance routes in apis/routes/_rest_routes_standard.inc.php invoke controllers without RestConfig::request_authorization_check(), so token ACLs are never enforced. Exploitation requires an enabled REST API and a valid bearer token; affected clients can expose patient documents and insurance data or alter documents and insurance assignments.
