Availability loss via missed shared PMD detection

PMD-sharing detection failure in Linux Kernel 6.13 and affected stable branches allows local users to cause availability loss via page migration. The hugetlb_pmd_shared function checks a page-table refcount that no longer tracks sharing, so migration proceeds on nonexclusive folios and pagemap marks shared mappings exclusive.
