Duplicate queue cleanup via default mremap

Memory-management error in the uacce driver in Linux Kernel allows local users to duplicate queue cleanup through a crafted mremap sequence. The uacce_vm_ops table omits an mremap handler, so the default operation copies vm_private_data into a second VMA and both VMAs invoke vma_close for the same queue. Reachability requires local access to a uacce mapping; the reported sequence frees q->qfr twice, but the second release is safe because the pointer is cleared.
