Account takeover via nonexpiring password-reset tokens

Insufficient reset-token expiration in Six Colors AB Hex.pm allows remote attackers with an old reset link to take over an account. Hexpm.Accounts.PasswordReset.can_reset?/3 checks only the key and primary email, omitting any comparison between inserted_at and the current time. Exploitation requires an unused reset link from the victim's current or historically exposed email; successful use changes the password and grants control of the account.
