Stored script execution via unescaped form name

Stored cross-site scripting in WPMU DEV Forminator Forms 1.50.2 and earlier allows remote authenticated users to inject scripts via form_name. The form-name handling stores the parameter without sufficient input sanitization and later renders it without adequate output escaping. Administrator-level access is normally required, but delegated form-management permissions can expose the same path to lower-privileged accounts; injected scripts execute when a user opens the affected page.
