Cross-patient appointment modification via IDOR

Improper access control in KiviCare before 4.5.4 allows remote authenticated users to modify other patients' appointments via IDOR. The appointment modification path accepts an appointment identifier from the request but does not verify that the current patient owns that record before processing cancellation or rescheduling. Patient-level access is sufficient, and the documented impact is unauthorized appointment changes rather than broader account takeover or code execution.
