Token disclosure via unrestricted backend address override

SSRF in HashiCorp Consul MCP Server 0.1.0 through 0.1.3 allows remote attackers to redirect Consul API traffic via a request header. The per-request address override accepts an unrestricted client-supplied destination and forwards configured Consul credentials to that endpoint. Exposure requires StreamableHTTP access by an untrusted client; credential disclosure additionally requires a Consul token in the server environment.
