Subscriber-level SQL injection via username placeholder

Blind SQL injection in 10Web Form Maker 1.15.44 and earlier allows remote authenticated users to extract sensitive database data. The dynamic-choice field code substitutes the current user's display_name for the {username} token and inserts it into an inadequately prepared WHERE clause. Exploitation requires subscriber-level access, a DB-backed choice field using that token, a payload saved as display_name, and a request to fm_reload_input.
