Unauthenticated code execution via unchecked upload filename

Arbitrary file upload in WebRehab Super Forms 6.3.313 and earlier allows remote attackers to write executable files through a form submission. In submit_form's PDF branch, datauristring bytes were written under the supplied value filename without type validation or path containment. Any unauthenticated visitor can obtain the required sf_nonce and session cookie, then upload executable content and run it under the web server account.
