Administrator-session script execution via unescaped form_id

Reflected XSS in CRM Perks Contact Form Entries before 1.5.3 allows remote attackers to run scripts in an administrator's browser. The leads page inserts the form_id query value unescaped into a JavaScript string in an inline script block, allowing the value to close the string and inject script. Exploitation requires an administrator to open a crafted link while authenticated, and the injected JavaScript executes with that administrator session's browser privileges.
