Cross-course Q&A disclosure via mismatched object authorization

Authorization bypass in Themeum Tutor LMS before 4.0.0 allows remote authenticated users to read and write cross-course Q&A threads. The tutor_qna_create_update AJAX action authorizes the supplied course_id but retrieves the separately supplied question_id without confirming that the question belongs to that course. Subscriber-level access to any course and a front-end nonce suffice to disclose another course's Q&A content and add a reply to its private thread.
