Administrator takeover via unverified OTP reset state

Authentication bypass in miniOrange OTP Login plugin 5.5.1 and earlier allows remote attackers to take over administrator accounts. The um_reset_password_process_hook() function trusts a public form_nonce and attacker-controlled username_b without binding either value to completed OTP validation. The Ultimate Member Password Reset Form integration must be active and phone-only reset disabled; a 302 response then discloses a reset URL for the targeted administrator account.
