Unauthorized cross-model access via unverified macaroon caveats

Improper authorization in Canonical Juju 2.9.x, 3.6.x, and 4.x allows remote authenticated users to retain revoked cross-model access. The controller accepts caveats from a macaroon it cannot validate after a missing root key or expiration, then uses those untrusted claims to mint a valid macaroon. A user able to update database records can impersonate an authorized user and keep consuming a cross-model offer after permission expires or is revoked.
