Subscriber-level auto-approved comments via unchecked AJAX target

Improper access control in Themeum Tutor LMS before 3.9.13 allows remote authenticated users to create auto-approved comments. The tutor_create_lesson_comment AJAX handler accepts an arbitrary comment_post_ID without authorization or target validation and inserts the comment with comment_approved set to 1. Subscriber-level access and a frontend nonce suffice; WordPress strips script tags but preserves permitted HTML links and bypasses moderation on any site content.
