Authenticated session access via unscoped title resolution

Authorization bypass in Nous Research Hermes Agent 0.12.0 and earlier allows remote authenticated users to resume another user's session. The resume endpoint passes a caller-controlled Title to resolve_session_by_title, which resolves a global session identifier without verifying ownership. A low-privileged account and a known or guessed session title suffice to access and continue that session without user interaction.
