Observation suppression via ambiguous hash input boundaries

Weak hashing in claude-mem 11.0.0 through 11.0.1 allows local users to suppress stored observations through hash collisions. computeObservationContentHash concatenates memorySessionId, title, and narrative without delimiters, so distinct field tuples produce the same truncated SHA-256 value. Low-privileged local access and carefully chosen field boundaries are required; a collision causes a legitimate observation to be silently deduplicated rather than stored.
