Physical device crash via stale USB root pointer

Use-after-free in Zephyr RTOS 4.4.0 allows physically proximate attackers to crash devices through repeated USB disconnect events. usbh_device_disconnect() frees the root usb_device slab object without clearing ctx->root, so dev_removed_handler() reuses the dangling pointer and frees the object again. Exploitation requires the default-off experimental USB host stack, an enabled UHC driver, and physical control of USB connection state; reallocated slabs permit live-object memory corruption.
