Service crash via unbounded DICOM sequence recursion

Stack exhaustion in Orthanc DICOM Server 1.12.0 through 1.12.11 allows local users to crash the service with a crafted DICOM file. DcmItem::read and DcmSequenceOfItems::read recursively parse nested sequence items without a depth limit until the worker thread exhausts its stack. Low-privileged local access and control of a file processed by Orthanc are required; the demonstrated consequence is interruption of all HTTP and DICOM services until restart.
