Local crash via out-of-bounds pragma read

Heap buffer overflow in NASM 2.17rc0 allows local users to crash the assembler by processing a malformed Mach-O pragma directive. In output/outmacho.c, macho_no_dead_strip duplicates a four-byte pragma string and then reads one byte past the allocated heap buffer. Triggering the fault requires local execution with Mach-O output selected; the published AddressSanitizer trace demonstrates a process abort, not code execution.
